Please download and fill out this form if your pet is scheduled to receive a Health Certificate for travel within the United States. Be sure to return the completed form to us before your pet’s appointment so we can work on your paperwork ahead of time and help make your visit as easy as possible.
International Health Certificate Pre-Visit Questionnaire
Use this form if your pet is scheduled to receive a Health Certificate for travel outside of the United States. In addition to completing and returning this form to us before your pet’s appointment, please be sure to do your own research on your destination country’s requirements.
